WASHINGTON -- So many things went wrong last Jan. 29 to contribute to the deadliest plane crash on American soil since 2001 that the National Transportation Safety Board isn't likely to identify a single cause of the collision between an airliner and an Army helicopter near Washington, D.C., that killed 67 people at its hearing Tuesday. Instead, their investigators will detail what they found that played a role in the crash, and the board will recommend changes to help prevent a similar tragedy.

In the early hours of 26 March 2024, the MV Dali departed the port of Baltimore bound for Sri Lanka. While navigating the Fort McHenry channel, the 1,000ft-long Singapore-flagged cargo vessel lost power before striking the bridge. The impact resulted in the deaths of six people who were working on the bridge at the time.

A vehicle operated by the FDNY was traveling on Avenue S when it was struck by a van heading north on West 11th Street in the Gravesend section of the borough. As a result, the FDNY truck then collided with an Access-A-Ride van, which ended up hitting a Lexus SUV, according to the NYPD.
There was a crash early Friday morning at the edge of Pacific Heights in which an SFPD vehicle collided with a car while it was chasing down a different vehicle. A boy driving a stolen car at excessive speeds hit and killed another motorist Tuesday morning in Oakley while trying to flee from officers, police said. Initially chased by Antioch officers, the car roared into Oakley at speeds of 80 mph around 10:25 a.m. at Main Street and Bridgehead Road, according to the Oakley Police Department.
The NYPD is investigating after a man was found dead aboard a Manhattan train on Sunday. According to police sources, cops from Transit District 4 were conducting an inspection of a northbound Q train just after midnight on Feb. 1, after it had pulled into the end of the line on East 96 Street and Second Avenue subway station, when they discovered a 34-year-old man unconscious and unresponsive.
A motorcyclist died Thursday after colliding with a sedan in what police said was a DUI-related wreck involving another driver. The crash happened about 4:45 p.m. in the area of Buchanan Road and Woodborough Place. Officers arrived to find the 46-year-old Concord resident down. He died despite rescue efforts by police and medical personnel.
A train collided with a car stuck on the tracks in Emeryville early Saturday morning, according to a police department press release. Shortly after 1 a.m., a woman reportedly turned onto the railroad tracks and became stuck. She exited the vehicle before an oncoming train hit it. The vehicle subsequently caught fire. Authorities stated that the railway was shut down temporarily so emergency response, fire suppression, vehicle removal and a full inspection of the tracks could take place.
Then in early December, another man was hit on the tracks by a train and killed in West Berkeley at Bancroft Way and Fourth Street. Now the Chronicle reports that on Monday, at nearly the exact same Bancroft Way and Fourth Street location, a man was hit by a train on those tracks and killed at around 11:37 am. As of Tuesday afternoon, the Alameda County Coroner's Office has not identified the victim.
