The Substance is distinctive less for its nutso, over-the-top gore than for a single scene midway through the film that exposes a different kind of body horroror, more specifically, the way insecurity can be its own kind of horror.
Moore's Elizabeth is a formerly beloved movie star who's now doing OK with her own 1980s-style exercise show, during which she leads a bevy of beauties in reasonably tasteful workout wear through a sweaty routine of stretching and jumping.
