McDonald's confirmed that contaminated slivered onions from Taylor Farms, used in affected burgers, came from a facility in Colorado Springs, prompting a supply chain overhaul.
The outbreak, which caused one death and numerous hospitalizations, led McDonald's to pull the Quarter Pounder from its menu until the source was identified.
