When things go wrong we search for a causal explanation...Active failures are exactly what we expect them to be; the errors, behaviors, settings, and choices that immediately cause a failure.
However, this is not the only immediate cause of failure...Latent failures occur months, sometimes years, beforehand, as design choices are made and implemented into the system.
