A printing error led to the potential disclosure of limited information that may have been sent to another Department of Veterans Affairs (VA) patient by mistake back in around mid-November 2023.
A total of 2,380 veterans in Pennsylvania, Ohio, New York, New Jersey, Delaware, West Virginia, Maryland, Virginia, Kentucky, and Delaware had to be notified of the potential disclosure.
