The California Department of Public Health found that violations at Ararat Nursing Facility were 'a substantial factor' in the deaths of two residents within three months.
In the first incident, a resident died after falling from a lifting device when left unattended; the facility now faces a $120,000 fine.
Another resident, who was poorly monitored during a meal, stopped breathing and died. Though family members aided her, the facility failed to ensure safe eating practices.
Despite these serious citations, Ararat Nursing Facility claims it is committed to the highest level of care and is appealing the state's decisions.
