The Lampard inquiry looking into the deaths of almost 2,000 mental health patients in Essex since 2000 excludes the case of Andrea Waddington, who committed suicide after going without a psychiatric appointment for three months.
Waddington, despite her struggles and eventual death, does not qualify for the inquiry as her case fell after the chosen time frame and because she was being treated in the community, raising concerns about the scope of the investigation.
