A Green Line trolley near Lechmere Station derailed after traveling more than three times the speed limit and ignoring a stop signal, according to NTSB.
The NTSB's preliminary report indicates that a two-car Green Line train exceeded the speed limit at 36 mph in a 10 mph zone before derailing.
The train, carrying about 50 passengers, derailed due to the lead truck continuing straight while the next trucks took a diverging track, leading to the accident.
Following the accident, the MBTA confirmed the train operator was speeding and remains out of service while the Safety Department investigates further.
