Inquiry into death of Aoife Johnston (16), who died after 12-hour A&E wait, will probe how UHL is run as HSE confirms damning findings

The HSE referred to an unpublished internal systems analysis report (SAR) into the tragedy - previously revealed by the Sunday Independent - that found overcrowding in the hospital was 'endemic'.

'There is little apparent understanding of the risks and inefficiencies caused to patient care by a crowded environment by the hospital system in terms of the impact on the emergency medicine doctors assessing and managing patients and the nursing staff's ability to provide safe care.
