Roy King, a convicted sex offender, died by overdose shortly after being accused of sexual assault in prison. Despite his history of self-harm, prison staff did not take necessary monitoring actions after the allegations were made. King's case highlighted significant deficiencies in prison protocols regarding self-harm and mental health monitoring, ultimately leading to the PPO's criticism of the lack of action taken by staff. An inquest ruled his death as misadventure, while investigations into the sexual assault allegation were closed posthumously by the police.
Mr. King had a history of repeated self-harm following inappropriate behaviour, when he lost his job or after staff told him to do something he did not want to do.
The PPO criticised a failure to adequately monitor King following the allegation, stating that staff did not consider starting ACCT monitoring immediately.
