Dena Collins, a 28-year-old primary school teacher, died from a heart attack caused by an undiagnosed deep vein thrombosis, despite visiting a hospital the day before. Symptoms included leg pain and fever, and although the hospital noted the possibility of a clot, she was misdiagnosed and sent home. Following her collapse the next day at home, she was taken back to the same hospital, where she died. The NHS Trust's serious incident report revealed that her death might have been prevented with earlier diagnosis and treatment.
