Without strong executive sponsorship, a project has no chance of survival. In PRINCE2, an entire process, IP (Initiating a Project), is dedicated to such crucial foundational matters. Without strong, engaged, and visible sponsorship, a project will fail. No exceptions. This may seem obvious but it's surprising how many projects start, and continue, with weak, ambivalent, or un-committed sponsors.
A clear Project Plan is fundamental for setting expectations with stakeholders. No plan means no guidance and a project without guidance goes nowhere.
Despite what 'agile' proponents, or those who decry 'perfectionism' might say, this quote by Dwight Eisenhower stands the test of time: In preparing for battle I have always found that plans are useless, but planning is indispensable.
The above failures sound ridiculous, but sadly, are not. They are real. A Project Plan is the quintessential artifact of project management. There are 3 types of failures here: Not knowing how to create a proper Project Plan, using inappropriate tools to create Project Plans, or not having a Plan at all (yes, this happens!), or having a Plan but not circulating or updating it.
