PSA: New Borderlands 4 Bug May Steal All Your Skill Points

Borderlands 4 is now out, and amidst several issues that players have reported, it looks like there's a bug that could cause you to lose all of your skill points. So what's causing this glitch to trigger? Gearbox says that disconnecting and rejoining a multiplayer session or a game crash in single-player could be the problem here, and it says that it has noted here.
