David Morais, 28, of West Roxbury suffered fatal injuries Tuesday while moving a stack of plywood at Materia Millwork in Avon. Emergency crews responded to a 911 call reporting an "industrial accident" just before 3 p.m.; Morais sustained a fatal head injury and was pronounced dead at the scene. Materia Millwork described Morais as a valued teammate, friend, and craftsman, expressing condolences to his family. State Police, Avon police, the Office of the Chief Medical Examiner, and the U.S. Occupational Safety and Health Administration are investigating the incident.
