Emma Simmonds, 41, attended the Queen Elizabeth Hospital early pregnancy unit after bleeding at almost 11 weeks and was told to return the next day, when she discovered she had miscarried. She previously sought gynaecological care for fertility struggles and underwent investigations for possible endometriosis. She was removed from a physiotherapy list for a missed appointment she says she was never notified about; the appointment belonged to another patient with the same name. She complained via the hospital Patient Advice and Liaison Service and reported no apology for the apparent data breach. The hospital was rated England's worst-performing acute hospital in new government league tables and the interim managing director issued an apology for failures to meet standards.
