The court heard Ali had been radicalised online by the group calling itself Islamic State and had previously been referred to the Prevent programme between 2014 and 2016. A governmentcommissioned review by Lord Anderson in 2025 found 'a string of failures' mainly attributed to 'human error'. Senior Essex coroner Lincoln Brookes declined the family's request to reopen the inquest, stating that shortcomings in Prevent did not of themselves amount to evidence regarding Sir David's death.
