"Mr Ahmed told the BBC his family had waited three years for answers about his brother's death. Mr Haidhar had travelled from his home in Barking, east London, into Leicestershire in the early hours of 23 August 2023. Shortly after 06:00 BST, he crashed his car into the central reservation - which was later found to have been caused deliberately. The 39-year-old was towed to the hard shoulder by National Highways officers, who spoke to Mr Haidhar's brother, Kaisor Ahmed, on the phone while they were at the scene."

Shafiqul Haidhar, a 39-year-old father of four with a history of mental health issues, was fatally struck by a lorry on the M1 near Lutterworth on 23 August 2023 after being towed to the hard shoulder following a deliberate crash into the central reservation. His brother, Kaisor Ahmed, repeatedly contacted police asking for him to be taken to a safe place and made a 999 call that connected to the Metropolitan Police. Call handlers missed opportunities to gather more information, a highways officer said motorway noise prevented hearing a plea, and a mental health trust did not identify earlier urgency. The coroner could not determine on the balance of probability that Haidhar intended to cause his own death.
