Alice Figueiredo, 22, died by suicide in July 2015 while detained on a mental health ward at Goodmayes Hospital after more than ten prior attempts. Staff failed to remove plastic items from communal toilets despite her known self-harming, creating an accessible means that contributed to her death. North East London NHS Foundation Trust was convicted of failing to ensure the safety of a non-employee and fined £565,000 with £200,000 costs. Ward manager Benjamin Aninakwa was convicted of failing to take reasonable care, given a six-month prison sentence suspended for 12 months and 300 hours unpaid work.
