An investigation into several adult deaths in and around Wiltshire began after an initial report in 2023. A man in his 30s from West Wiltshire was arrested in June 2024 on suspicion of six counts of gross negligence manslaughter and four counts of ill-treatment or wilful neglect by a care worker. A 59-year-old woman was arrested on suspicion of gross negligence manslaughter in March 2025. Both South Western Ambulance Service workers have been released on bail. The ambulance service started an internal investigation immediately, relieved the staff of duties, and said one individual is no longer employed. Police stated the investigation focuses on supporting families. The Crown Prosecution Service definition of gross negligence manslaughter requires a breach of an existing duty of care that gives rise to a serious and obvious risk of death. The service was most recently inspected by the health watchdog.
