In the 1980s, a training instructor named Steven experienced an embarrassing mistake during a session at Intergraph. While demonstrating to students, he accidentally powered down a VAX server instead of a tape drive due to the poor ergonomic design of the buttons. This caused all systems to halt, but Steven chose not to inform the students about the mishap. He managed to leave the machine room without taking responsibility for the error, claiming innocence and reflecting on the situation many years later.
In a triumph of ergonomic design, the VAX power button and the tape power button were located right next to each other.
Suddenly, all the training systems just stopped dead as I power cycled the VAX that supported all the terminals in the training centre.
I walked out of the machine room claiming innocence and got away with it.
