Baroness Amos will lead a national investigation into failures in NHS maternity and neonatal services across 14 trusts in England. Bereaved families will be central to the inquiry to drive improvements after systemic problems spanning more than 15 years. Health Secretary Wes Streeting acknowledged the courage of bereaved families and said their strength can help protect other families. Independent reviews found failings such as women's voices being ignored, safety concerns overlooked, poor leadership and toxic cultures. A range of services and the 14 selected trusts will be placed under scrutiny to identify and address preventable tragedies.
