Staff described a keeping-your-head-down culture where concerns were inadequately addressed. Findings were given to trust leaders in January but were not shared widely with staff until ten months later, delaying urgent action on 19 recommendations. Multiple serious incidents occurred, including arrests and six staff imprisonments: nurses jailed for stealing drugs and unlawfully sedating patients, and a leading heart surgeon imprisoned for sexually assaulting colleagues. A coroner ruled that an elderly female patient was sexually assaulted and unlawfully killed in her hospital bed; a murder investigation concluded without identifying a culprit.
