A five-year-old boy at Danetree Primary School lost the tip of his finger after it was trapped in an unsupervised toilet door lacking hinge guards.
HSE principal inspector Emma Stiles emphasized the incident could have been avoided with the installation of inexpensive hinge guards, suggesting a widespread review by schools.
