Two elected leaders propose adding a Valley Homeless Healthcare Program team to visit San Jose shelters and provide mental health and substance use services starting later this year. The proposal would use existing county budget funding pending Board of Supervisors approval. The plan creates a single coordinated referral and intake system so staff can assign shelter beds across city and county sites and refer clients to county treatment. County and city currently operate separate temporary housing sites and intake systems; the coordinated entry aims to close gaps, increase visibility of placements, and better deploy limited behavioral health resources.
