Ms Rawden outlined how permission was still given for escorted leave two days before Mr Leadbeater was attacked, when staff at Greater Manchester Mental Health NHS Foundation Trust failed to follow their own policies and did not have an accurate risk assessment."

A 74-year-old special needs bus driver, Roger Leadbeater, was stabbed multiple times while walking his dog in a Sheffield park in August 2023 by Emma Borowy, who had absconded from leave from an acute mental health unit in Bolton. The coroner found Borowy had absconded nine times, attempted 15 absconds and failed to return from leave three times. Staff at Greater Manchester Mental Health NHS Foundation Trust granted escorted leave despite not following policies and lacking an accurate risk assessment. The coroner concluded proper procedures would likely have prevented the leave. The coroner also criticised Greater Manchester and South Yorkshire police handover procedures. Borowy died in prison four months after the attack and the victim's family said the health and police trusts failed on every level.
