A litany of "missed opportunities" are revealed in a confidential report investigating the hospital care of Bryonny Sainsbury, who died just days after sustaining a brain injury following a kick from her horse.
The 25-year-old from Briskil, Newtownforbes, Co Longford, died in Beaumont Hospital, Dublin, on August 31, 2021 after being kicked by her horse and hitting her head against a wall.
