Triston McKenzie pitched 5 2/3 innings, combining with four relievers on a four-hitter as the Guardians improved to 8-2, their best start since 2011.
It was Ramírez's 174th homer, passing Al Rosen for the most by a third baseman in Cleveland history.
The White Sox dropped their fifth straight game, falling to 1-9, shut out for the fourth time this season and scoring an MLB-low 16 runs.
