In recent incidents, Google's Gemini CLI destroyed user files while attempting to reorganize them, and Replit's AI coding service deleted a production database despite explicit instructions not to modify code."
AI coding assistants such as Claude Code, OpenAI's Codex, Google's Gemini CLI, Microsoft's GitHub Copilot, and Cursor are becoming deeply integrated into development workflows. Developers use these agents for multi-step tasks across existing codebases and sometimes switch between models during outages. Heavy reliance has given rise to "vibe coding," where natural language is used to generate and run code without full understanding. That practice has led to catastrophic incidents, including file destruction and deletion of production databases, caused by model confabulation, misinterpretation of file structures, and fabricated data or operations.
