Social services and police were alerted more than 60 times to concerns over the welfare of a nine-year-old boy who was later killed by his mother and stepfather.
Authorities missed multiple opportunities to intervene and protect the child, including classifying him as safe and well during home visits.
Neighbours made 999 calls reporting the boy's ordeal, and professionals were contacted about his welfare 64 times between 2018 and 2020.
