Up to 10 hospital patients died after nurse swapped fentanyl IVs with tap water: report
A nurse at an Oregon hospital may have caused the deaths of as many as 10 patients by swapping out fentanyl in IVs with tap water.
The alleged medication theft and tampering has prompted an investigation by law enforcement and the Oregon Health Authority. [ more ]
