Fiona Wilcox, Senior Coroner for Inner West London, issued a Prevention of Future Deaths report to Health Secretary Wes Streeting, in which she said there were not enough beds or nurses to manage demand in A&E in both cases. She warned that while hospitals like St George's had put in place measures to try to manage demand, A&E departments were still exceptionally busy and a risk of further deaths remained.

A Berkeley man has died more than a month after he suffered a head injury when he crashed his scooter on the Cal campus, authorities said. UC Berkeley Police said the man was found unconscious on Dec. 1 near Moffitt Library in the center of campus, after witnesses said he fell off his scooter at high speed and hit his head. The man was unconscious but breathing at the time, Sgt. Jacob Westlie wrote in an email, and was taken to a hospital.

Brian Mitchell, 72, was killed by a train terminating at Stratford on 26 December 2023. The emergency brakes were not applied by any of the four incoming train operators, the inquest into his death heard. Mitchell had spent almost an hour at the end of the platform before he fell on to the track. He had 272mg of alcohol in his system, more than three times the legal driving limit.

A man who was found dead on the Bridgeview Trail in Oakland's Oakmore neighborhood died by suicide, the Alameda County Coroner's determined after an investigation. The 36-year-old man, who The Oaklandside is not identifying, was found unresponsive by a parkgoer on June 30 just off the popular trail, which winds along a hillside above Sausal Creek in Dimond Canyon. Oakland police, firefighters, and paramedics with Falck responded to the scene and cut the man down from a tree, according to police records.
