PEPFAR, launched in 2003, has saved over twenty-six million lives, improved maternal and infant health, and enjoyed bipartisan support. By 2024 it was described as possibly the single most successful U.S.-Africa policy. In January 2025 the Trump administration froze PEPFAR operations, producing a 100-day fallout observed in April 2025 through 39 interviews with healthcare workers, patients, experts, and NGO staff in Tanzania and Uganda. The fallout includes growing antiretroviral shortages, babies being born with HIV, mothers contemplating or undergoing unwanted abortions due to fear of transmission, and increased stigma and harm toward LGBTQ+ communities.
