A 73-year-old woman, Sue Howell, died from a pulmonary embolism after being discharged from New Cross Hospital. Test results that would have indicated a blood clot were available on a computer system but were not acted upon by staff. The Black Country assistant coroner concluded that neglect in medical treatment contributed to her death. A doctor gave evidence that she did not know the D‑Dimer test had been requested and that the result was not in the patient notes despite being available hours before discharge. The hospital apologised and reported that an investigation led to several actions. Her family expressed anger and ongoing distress.
