Miss J Alexander served as runway coach on America's Next Top Model from its first season in 2003 and was a judge from season five through season 13. He continued as a runway coach on the show's international spin-offs after 2012 and the series ended in 2018. On 27 December 2022 he was hospitalized after suffering a stroke, spent five weeks in a coma, and emerged unable to walk or speak. He expressed that he cried and that he misses teaching models on the catwalk. The documentary Reality Check was filmed in early 2025. As of February 2026 he remained unable to walk but expressed determination to recover.
