Microsoft Confirms Windows Update Bug Blocking Security Fixes
"This issue occurs on devices that have limited free space on the EFI System Partition (ESP), especially if it has 10 MB or less available."
"It also says that some affected users may have their update pass the initial stage, but fail to complete, with the error occurring "during the restart phase at approximately 35-36% completion.""
"Affected users will have their updates rolled back and may see a message with the lines "Something didn't go as planned. Undoing changes," and will be directed to check the log entries at C:\Windows\Logs\CBS\CBS.log, which indicate a lack of free ESP space as the cause."

KB5089549 fails to install on some Windows 11 devices with limited free space on the EFI System Partition. The failure commonly occurs mid-install and can trigger an automatic rollback, leaving devices unpatched. Affected systems may show error 0x800f0922 and may fail during the restart phase around 35–36% completion. Microsoft has acknowledged the issue and deployed a Known Issue Rollback to prevent the problematic update experience. Devices that already attempted the update may display a message indicating changes are being undone and may direct users to check CBS.log for evidence of insufficient ESP space. Microsoft provides a workaround through the KIR feature for devices that did not receive the update successfully.
