Maddy Cusack, a 27-year-old midfielder for Sheffield United, was found dead at her home in Derbyshire in September 2023. She had played for the Blades since 2019 and became the first player to make 100 appearances for their women's side. Her sister Olivia has a wrist tattoo in Maddy's memory and the family established a foundation to support women and girls in football. An inquest opened in October 2023 but has been delayed and is now expected in January, leaving unanswered questions. An external club-ordered report cleared the club, and an FA report will be published after the inquest concludes.
