BART experienced a systemwide meltdown just before the morning commute, the second major failure in four months. Contract workers moving fiber-optic cables at Montgomery Station during a train-control equipment upgrade caused the malfunction. Assistant General Manager Sylvia Lamb accepted responsibility, calling the event avoidable and saying her team underestimated the risk and its impact. The team acknowledged that isolating the west and east branches could have allowed a quicker restart. Eight similar cable switches occurred without incident, while 41 more stations still require replacements as part of the long-running upgrade. BART is working with its vendor to prevent future outages and the board also addressed a separate Transbay Tube smoke incident.
