Josiah Harrell received an unexpected UFC call to replace Sean Brady at UFC 290 after Brady withdrew with a staph infection. Harrell rushed a weight cut and medicals, during which brain scans revealed moyamoya, a rare condition that restricts cerebral blood flow. The UFC canceled the scheduled fight and terminated Harrell's contract. Harrell responded with gratitude, recognizing lifelong stroke and seizure risks tied to high blood pressure and describing how the diagnosis strengthened his faith and sense of purpose. Harrell returned to competition in LFA 224 riding a three-fight win streak and facing Bekmyrza Dosmatov.
