Data from 432 transgender patients aged 12 to 20 who initiated gender-affirming hormone therapy between 2017 and 2024 at a Midwestern academic medical center show substantial declines in suicidality. Patients completed Ask Suicide-Screening Questions (ASQ) prior to hormone therapy and again at follow-up visits. Average follow-up occurred around two years after treatment initiation, with some responses up to five years later. Suicidality scores among respondents declined by over 67 percent and the reduction was consistent across sex assigned at birth, age at therapy start, and treatment duration. Seven patients discontinued therapy for various reasons, including identity shifts and hair loss concerns.
