A 41-year-old nurse from Bakersfield presented for planned removal of a 22-pound benign ovarian cyst at Cedars-Sinai Medical Center. Routine preoperative testing revealed pregnancy, and three days later abdominal pain prompted MRI, ultrasound, and blood tests. Imaging showed an ectopic abdominal pregnancy with the fetus located near the liver and the uterus empty; the large cyst had displaced organs. The pregnancy posed high risks of maternal bleeding and fetal death because the placenta could not grow safely. A multidisciplinary surgical team of about 30 specialists removed the cyst and delivered a healthy baby boy in a complex, successful operation.
