The Federal Bureau of Prisons issued guidance in February requiring the reduction and discontinuation of hormone therapy for transgender inmates. The policy prohibits providing hormone therapy to inmates who were not receiving it before incarceration and mandates plans to taper treatment for those already receiving it. Medical experts, including Dr. Carl Streed, warn the policy contradicts evidence-based care standards and creates different treatment standards for incarcerated people versus the general community. Discontinuing hormone therapy causes serious health risks including cognitive and mood changes, increased cardiovascular disease risk, and metabolic issues. For inmates who underwent gender-affirming surgeries, risks are more severe as their bodies no longer produce adequate natural hormones. Legal experts contend the guidance violates a federal judge's preliminary injunction challenging the administration's anti-trans prison policies.
