Bruce Willis remains physically mobile and in generally strong physical health, while experiencing progressive cognitive decline concentrated in language and communication. An aphasia diagnosis in 2022 affected his ability to speak and prompted retirement from acting. Less than a year later, a diagnosis of frontotemporal dementia clarified the neurodegenerative progression, which brings inevitable decline in functioning. The family has adapted by developing alternative ways to communicate. Occasional moments of his former expressive personality still appear, such as a visible twinkle or spark, but those moments are brief and often fade quickly.
