Assistant General Manager Sylvia Lamb accepted responsibility, calling the outage avoidable after a cable move between switches triggered a rapid loss of network connectivity and a "network storm." The outage was the second systemwide incident in four months and the third since 2019. Directors demanded accountability and consideration of hiring outside expertise to prevent future failures. General Manager Robert Powers acknowledged concern and said the agency will seek external help if needed. Executives also referenced an unrelated Transbay Tube smoke incident under investigation.
