TeamCare Medical opened a new primary care facility at 629 Park Place in Crown Heights with a ribbon-cutting attended by healthcare leaders, elected officials and community partners. The facility implements a whole-person model that integrates medical services with social supports under one roof. The first floor houses primary care and aligned specialty services while the second floor will operate as a social daycare program to keep older adults engaged, socialized and supported. The program emphasizes collaboration with community practices and community-based organizations to create an accessible, neighborhood-focused model of care that builds on experience from the PACE program.
