"From a medical point of view, this is alarming because it's essentially saying that a form of evidence-based care will no longer be provided to people under the purview of the Bureau of Prisons. That means the policy runs counter to best practices and arguably probably the law in terms of providing care to inmates because it's setting up a different standard for them versus the standard out in the community."
The Federal Bureau of Prisons issued guidance on February 19 prohibiting the initiation of hormone therapy for incarcerated transgender people and requiring clinicians to develop tapering plans to discontinue treatment for those currently receiving it. Medical professionals characterize this directive as a political decision that contradicts evidence-based clinical practice and established standards of care. The policy creates a different treatment standard for incarcerated individuals compared to the general population, potentially violating medical ethics and legal obligations to provide appropriate care to inmates. Physicians emphasize that hormone therapy is essential medical treatment for gender dysphoria, not cosmetic medicine, and that discontinuing it could cause immediate psychological distress and long-term medical harm to a vulnerable population.
