The deficiencies detailed in the review included issues with line of sight that were acknowledged but not adequately addressed. Rowe added, "At approximately 18:10 local time, the Secret Service security room called the countersniper response agent about an individual on a nearby roof, but that vital piece of information was not relayed over the radio network, demonstrating systemic breakdowns that endangered lives."
