Jordan McSweeney, a sexual predator with 28 previous convictions, killed Zara Aleena while on parole, despite previous violent offenses in prison not being shared.
Multiple reports of violent offenses by McSweeney in prison were not effectively communicated, allowing him to be released and subsequently murder Zara Aleena.
