Failures across multiple agencies contributed to the death of Zara Aleena.
Jordan McSweeney was jailed for life with a minimum term of 33 years for Ms Aleena's rape and sexual assault, having been released from prison nine days earlier.
Probation report in January found McSweeney was not categorized as a high-risk offender when he should have been, highlighting system flaws.
