The National Transportation Safety Board concluded that faulty engineering produced a carbon-fiber composite pressure vessel with multiple anomalies that failed to meet required strength and durability standards. OceanGate did not adequately test the Titan, leaving the company unaware that the pressure vessel's actual strength and durability were likely much lower than intended. OceanGate's real-time monitoring analysis was flawed, preventing recognition that the Titan was damaged and needed removal from service after a previous dive. The debris search could have been faster had standard emergency-response guidance been followed, saving time and resources. The Titan imploded about 1 hour 45 minutes into a 2023 Titanic expedition, killing all five people aboard.
