A vulnerable passenger fell onto the London Underground track at Ickenham station on 28 March 2025 and was struck by a train at 22:30 GMT. CCTV footage showed the passenger was aware of the approaching train but could not reach safety because communication cables blocked the under-platform recess. The passenger lay undiscovered for two minutes. Contributing factors included platform lighting creating contrast with the darker track bed and the passenger wearing dark clothing, reducing visibility. The RAIB investigation made three recommendations to TfL: review safety standards for under-platform recesses, complete platform-specific risk assessments, and identify measures to reduce platform-fall risks. A coroner noted two years later that risks of fatal harm remained unmitigated.
